Core BenefitsThis is a brief summary of benefits available to full-time and part-time eligible hourly employees; the costs for these benefits are shared between the hospital and the employee. In all cases the Summary Plan Description will define eligibility and benefit limitations.
MEDICAL
Coverage is available to employees and their covered dependents through a PPO plan which offers three tiers of coverage. Our extended network covers facilities in both the Louisville, KY and Nashville, TN areas, providing our employees with access to the best healthcare available.
HEALTH REIMBURSEMENT ARRANGEMENT (HRA)
This is a newly added feature to our medical plan and is fully funded by Methodist Hospital. These funds can be applied to deductibles and coinsurance, or can be rolled over for future use, even into retirement.
DENTAL
Coverage is available to employees and their covered dependents through a large dental network. All dental specialties are included. We offer a basic and premium plan. The premium plan provides coverage for extras, such as cosmetic bleaching and orthodontics for children.
VISION
Coverage is available for both employees and their dependents, covering annual exams, lenses, frames, and contacts. This plan also provides limited coverage for Lasix.
DISABILITY
Plan (both short-term and long-term) allows employees to receive a percentage of base pay as income while unable to work.
LIFE INSURANCE
is provided to the employee at no cost. The employee can purchase supplemental life coverage for himself, and can also purchase coverage for his or her spouse and/or dependent children at a group rate.
ACCIDENTAL DEATH & DISMEMBERMENT (AD&D)
Insurance can be purchased as an individual or family plan at a group rate.
FLEXIBLE SPENDING ACCOUNTS
are available for both Medical and Dependent Care. A debit card makes the Medical Spending Account easy to use.
403(b) PLAN
allows the employee to defer pre-tax contributions toward their retirement. The hospital will match 60% of the first 6% of salary contributed.
RETIREMENT PLAN
In addition to the money matched in the 403(b) Plan, the hospital fully funds a retirement plan for all employees who work a minimum of 1,000 hours each plan year.
